Our Health Insurance plan covers you and eligible participants to see providers on the MULTI PLAN PHCS Network.
To locate providers participating in the Multiplan PHCS network, call (888) 263-7543 or go to www.multiplan.com and click "Find a Provider" located on the top-right hand corner of the page and follow the steps below
- After acknowledging that you have read the disclaimer at the bottom of the screen, click on the bottom "Select Network" button.
- Enter one of the search criteria suggested in the search box to begin your search.
- When selecting your network, choose "PHCS", then "Limited Benefit Plan".
- If your browser settings don't allow your location to be detected, enter a zip code.

2. 🦷 Dental Insurance 🦷
CareSphere offers a full Dental Insurance package to its employees and their dependents for a minimal fee per week.
How to find a participating Dentist in the network:

Provides cash directly to you to help with costs associated with an injury. These include initial visits and follow-ups, hospitalization, surgeries, MRIs, physical therapies, crutches, burns, stitches, and also an annual wellness benefit of $60.

In addition to our weekly pay option, CareSphere offers a daily pay option as a benefit to our employees. We understand that different individuals have various financial needs and preferences. Our weekly pay option provides the convenience of receiving your earned wages on a weekly basis, helping you budget and manage your finances effectively.
9. 👕Discounted Scrubs 👕****
CareSphere has scrubs available to purchase, as well as other important commodities such as gloves, masks, etc. HHAs are encouraged to wear scrubs during all assigned shifts. You will receive one complimentary scrub top as part of your welcome packet if you are a new hire starting after July 1, 2024. Additional scrub tops can be purchased at a discounted price of $20 each, with shipping directly to your home. Please contact the HR department to arrange for the purchase
and delivery.
Additionally, gloves are available free of charge to all caregivers.

10. 💲 Extra Holiday Pay 💲
Employees will receive an extra $3 per hour worked on CareSphere recognized holiday.
11. 🧳 Paid Time Off 🧳****
Our employees enjoy the benefit of paid time off!
For every 30 hours worked, employees accrue 1 hour of paid time off. This allows our team to rest and recharge, ensuring a healthy work-life balance while rewarding their hard work.
12. 💵 Employee Referral Bonuses 💵
We are constantly seeking additional PCA's, LPN's and HHA's.
To qualify for bonus pay, please adhere to the following guidelines.
1. You will eligible to receive a bonus when the referred employee successfully completes a total of 160 hours of work with CareSphere within the initial 3 months of their hire.
2. To be eligible for the bonus pay, you must remain actively employed and working shifts with CareSphere at the time when the bonus payment is scheduled to be disbursed.
3. Please note that each referral can only be claimed by a single individual. Duplicate claims for the same referral will not be honored.
13. 💵 Patient Referral Bonuses 💵
Each bonus pay whenever your referred patient completed 2 weeks of service.
1. If the patient receives up to 20 hours of service per week, you will be eligible for a $25 bonus.
2. For patients receiving 21 or more hours of service per week, the bonus amount increases to a total of $50 for 2 weeks.
To qualify for this bonus, you must be actively engaged in a patient's case withu s at the time the patient completes their second week of service. Thie program is designed to reward your dedication and commitment to providing quality care and referring patients who require our services.
