Progress Notes

This procedure applies to Licensed Practical Nurses (LPN) Only!

In addition to using EVV, all LPNs are required to submit a Progress note when finishing the shift.
NO LPN SHIFT IS CONSIDERED COMPLETE WITHOUT A PROGRESS NOTE.

How to submit a progress note:

1. Progress notes must be submitted before Clocking out for the day.
2. Click this link to open the progress note on your phone/Computer
   • If you do not have access to the internet, it can also be completed in paper format. The paper progress note is attached to this article for you to print.
3. Complete the form and submit it.

Note: Visits will not be paid without a completed progress note
